Community Care Services

Our community care services provide community-focused health support at the Y, including blood pressure checks, mobile healthcare visits, monthly health chats led by experts, and dietitian-designed meal pickup. Offered in partnership with local organizations, these services are available to both YMCA members and the broader community, with select services reserved for members.

Blood Pressure & Glucose Screenings

OSF Healthcare offers free blood pressure and glucose checks in the main lobby area at the YMCA. These quick, convenient screenings are a great way to monitor key health indicators, track your progress, and ask questions about your results.

Available for free to Streator Family YMCA members. 

Next blood pressure & glucose screening dates:

  • Tuesday March 10th 9:00-10:00 AM
  • Tuesday March 31st 9:00-10:00 AM

OSF King Care-A-Van

The OSF King Care-A-Van visits the Streator Family YMCA on the third Tuesday of each month from 9:00 AM–12:00 PM, offering free community health services right at the Y.

Available services include:

  • Health screenings (blood pressure, pulse oximetry, blood glucose, and cholesterol)
  • Health care navigation and support
  • Technology assistance with OSF MyChart
  • Connections to community resources such as food and transportation 
  • & More!

Open to the public. No appointment needed.

Wellness Chat Tuesdays

Join us for a monthly wellness chat on the second Tuesday of each month from 11:00 AM–12:00 PM in our Action Arcade, led by experts such as our Wellness Coordinator, OSF Healthcare staff, and other guest professionals. These sessions cover a variety of wellness topics, offer practical tips, and provide time for questions.

Available for free and open to the public.
